If you are a holder of a blue or green insurance card, you can set up an account in the Moje VZP app and be able to report changes or the loss of your card, submit an overview for self-employed persons, or request a contribution without having to go to a branch.
How do I register and what do I need to do so?
Via online form
Open the online registration form and follow the instructions to complete it.
After submitting the form, you will be able to log in to your account, but the information will be anonymised, mainly for privacy reasons. You must activate your account to view the required information. You can perform the activation:
- by inserting the Activation Key, which will usually arrive by registered mail to the address you provided in the registration within a few days (at the latest within 14 days) of submitting your registration application; after logging into Moje VZP, enter the key in the appropriate field
- or you will be asked to visit any branch, where we will activate your account on the spot on the basis of the identity document presented.
