Conditions for drawing

General conditions

  • contributions are provided from 1 February 2024
  • a contribution from the prevention fund may only be obtained by a person insured with VZP who meets the conditions set out in the given programme and is not in arrears with any premiums, penalties or accessions for health insurance (including where the insurance payer is not known)
  • persons who are exempted from the public health insurance system or for whom VZP is only an auxiliary health insurer in the health insurance system and thus have a yellow insurance card cannot draw from this contribution
  • when applying for the contribution, the insured person must not have been re-registered with another health insurance company when the contribution is drawn
  • you can apply for this contribution 2 times a year until the limit is exhausted (the exceptions are the contributions for Diet Programmes (once per half-year) and to Improve the Availability of Psychosocial Support (also possible after each session) and for Vaccination (here it is already exhausted in the first application)
  • we accept payment documents with a date of payment after 1 January 2024, which at the time of application must not be older than 3 months from the date of payment (where there are multiple payment documents, they must not be older than 3 months from the date of payment of the last payment document)
  • the minimum contribution amount per application is CZK 100
  • applications with payment vouchers paid after 1 January 2024 are recognized for the 2024 prevention fund contribution, meaning they cannot be used for the 2023 contribution
  • the rules and conditions for granting the contribution, in particular the age limit and the document age, apply as of the date of application, unless otherwise indicated for a specific contribution. For vaccination contributions, the date of vaccine administration is decisive
  • in the calendar year in which the insured person reaches the age of 18, the contribution for children up to the age of 18 and adults aged 18 and over cannot be combined within the same contribution category
  • payment documents can be aggregated up to the limit set for the specific contribution. The age of payment documents is assessed according to the date of the last payment for the goods, services or activities for which the contribution is allowed
  • there is no legal entitlement to contributions from the prevention fund, and their offer does not automatically create an entitlement to draw them
  • the contribution will not be provided for activities or products that do not clearly meet the preventive nature of the individual programmes


What the contributions do not cover

  • payment documents for rentals (e.g. halls, gyms, courts, saunas, swimming pools, hot tubs, solariums)
  • payment documents for on-line activities/video courseson-line sports/rehabilitation exercises/programmes
  • any shortfall in the reimbursement from public health insurance and the payment to the provider of the service/good (e.g. health services, medicines and medical devices covered by public health insurance)
  • payment documents that have already been used in the current year to draw a contribution from another programme (one payment document cannot be used to draw contributions from multiple programmes)
  • payment documents for multiple people where it is not possible to identify the amount of money for each individual (e.g. family season tickets)
  • activities covered by the Multisport card
  • ambiguous, illegible, altered or in any way defaced documents


Who can apply for a contribution

  • a person insured by VZP who is over 15 years of age; a legal representative of a person insured by VZP who is under 18 years of age and who presents the necessary documents, usually the child's birth certificate
  • an authorised person - on the basis of a power of attorney, which does not have to be based on the specified template and notarised, but must contain all the requisites
  • the institution to which the child has been entrusted; this applies in particular to children's homes, while the staff member, when applying for the contribution, must provide evidence that the child has been entrusted to it and the authorisation for them to act on behalf of the institution
  • an adoptive parent, guardian or tutor who provides evidence of the relevant court decision on the custody of the child
  • a foster parent who, in addition to the court order, also provides the consent of one of the child's legal representatives; this may be replaced, if necessary, by an affidavit from the foster parent


When you can apply for a contribution

  • in the Moje VZP application from 1 February 2024 to no later than 31 December 2024
  • other methods of submission from 1 January 2024 to no later than 30 November 2024

For gluten-free and low-protein diets from 1 January 2024 to 30 June 2024 (for the first half-year) and from 1 July 2024 to 31 December 2024 (for the second half-year)
If you submit an application through a holder of a postal/transport licence, by placing the application in a collection box or mailbox of a branch or by handing it over at the VZP mailroom, the date stated on the stamp is decisive.


How you can apply for a contribution

In the case of the contribution for rehabilitation activities linked to a screening examination and the contribution for dental hygiene conditional on an examination by a general practitioner, this is the only way you can apply.
If you do not already use Moje VZP, it is easy to register:

  1. Open the on-line registrationform and follow the instructions provided to complete it, or log in using Identita občana.
  2. We will then send you a registered letter that will include your authentication data. Only then will you be able to use the application. If you do not want to wait, you can register at any VZP branch.
  3. You can apply for a contribution without having to fill in your personal details.

Parents can apply for their children after registering them in their own created and registered Moje VZP account.

  • by visiting any VZP branch in person - in this case you only need to bring the required documents with you, and do not need to fill in the Contribution Application
  • by placing the Contribution Application together with all the required documents in the branch collection box/mailbox or by handing it in at the VZP mailroom
  • by post - by sending the Contribution Application together with all the required documents to VZP ČR, Regionální pobočka Praha, Na Perštýně 359/6, 110 00 Praha 1


What you need to submit when applying for a contribution

  • through the Moje VZP application
    • a scan or photograph of the payment document for payment for the service, goods or activity with all the necessary requisites; a scan or photograph of other required documents indicated for specific contributions (e.g. a doctor's certificate of diagnosis, vaccination certificate, pregnancy certificate, etc.)
  • by post
    • the child's birth certificate, if you are applying as a legal representative of a person insured by VZP under 18 years of age,
    • an affidavit by a foster parent, a power of attorney
    • the relevant court custody order if you are applying as foster parents, guardians, adoptive parents or tutors and the institution to which the child was entrusted
    • a completed and signed application from the prevention fund 2024
    • a payment document for payment for the service, goods or activity with all the necessary requisites, other required documents indicated for specific contributions (e.g. a doctor's certificate of diagnosis, affidavits, vaccine certificate, pregnancy certificate, etc.)
  • in person
    • ID card (to verify identity),
    • the child's birth certificate, if you are applying as a legal representative of a person insured by VZP under 18 years of age, 
    • an affidavit by a foster parent, a power of attorney
    • the relevant court custody order if you are applying as foster parents, guardians, adoptive parents or tutors and the institution to which the child was entrusted
    • a payment document for payment for the service, goods or activity with all the necessary requisites
    • other required documents indicated for specific contributions (e.g. a doctor's certificate of diagnosis, affidavits, vaccination certificate, pregnancy certificate, etc.)


Requisites and forms of payment documents

  • the contribution is provided during the duration of the programme exclusively against documents proving payment for the activity, service or goods in Czech crowns
  • a payment document is considered to be an invoice, sales slip, receipt, cash payment document, confirmation, etc. If you submit a copy, scan or photo of a document, the VZP may request the original of the payment document within 90 days from the date of application
  • we accept payment for services, goods or activities using any type of voucher, including benefit cards (the purpose and amount of the payment must be clear from the document submitted)
  • the document must be easily legible, especially if it is a scan or photocopied document for an on-line application
  • the payment document must include:
    • the name of the submitted document
    • the scope and object of the performance, i.e. the name of the service, goods or activity. In justified cases, the client may supplement this information (including their signature)
    • the date of issue or payment of the document
    • the price in CZK
    • identification of the company (company ID number, name, address - written, printed or using a stamp). The VZP reserves the right to check submitted payment documents against the activities listed in the public register for the supplier that issued the payment document
    • identification of the insured person for whom the contribution is intended. In justified cases, the identification of the person may be supplemented by the applicant, including their signature on the back of the document. This does not apply for payment documents like sales slips/receipts etc. (so-called simplified payment documents) that are issued by the provider without a name. These are payment payment documents from pharmacies, drugstores, etc. For these documents, the identification of the person for whom the performance is intended is not required.
    • payment documents where payment was made in cash must clearly show that the payment was made
    • in the case of payment documents where payment was made by wire transfer, payment card, cash on delivery or through a payment gateway, and the payment is not clearly visible from the payment document, a bank account statement (or a confirmation issued by the bank, an excerpt from a bank account statement, a document from a payment terminal or a cash payment document) is required

How you will receive your contribution

  • on the bank account specified on the contribution application
  • by postal order (in exceptional cases - this cannot be used when applying for a contribution via the Moje VZP application)

If the contribution is requested by an authorised person on the basis of a power of attorney, the contribution is paid onto a bank account (or sent by postal order to the address) in accordance with the details specified in the power of attorney.
If the client provides an invalid account number on the application or fails to collect the funds sent by postal order in time and the amount is subsequently returned to the VZP account, the VZP is not obliged to contact the client and notify them of this fact.
The VZP reserves the right to modify the programmes during the year or terminate them if the financial limit is exhausted.
