Conditions for Drawing Contributions from the Prevention Fund for 2026

Please always read carefully the conditions of the contribution you are interested in, as well as the general conditions for obtaining contributions from the Prevention Fund.
Please also familiarise yourself with the information provided in connection with the processing of personal data when drawing funds from the Prevention Fund.


How to Apply and Within What Period

You can submit your application:

  • via the Moje VZP web or mobile application from 1 February to 31 December 2026, or
  • in person at the counter or mailroom of any VZP branch office from 1 January to 30 November 2026, or
  • by post to the address:
    VZP ČR, Regional Branch Prague, Na Perštýně 359/6, 110 00 Prague 1,
    from 1 January to 30 November 2026.

For gluten-free and low-protein diet programmes, applications may be submitted:

  • from 1 January to 30 June 2026 (for the first half of the year) and
  • from 1 July to 31 December 2026 (for the second half of the year).

If the application is submitted by post (for example via Czech Post, s. p. or another carrier holding a valid postal or transport licence), the decisive date is the one indicated on the postal stamp of the carrier.
If the application is handed over directly at a VZP mailroom, the decisive date is the one indicated on the VZP mailroom stamp.


General Conditions

  • A contribution from the Prevention Fund may only be granted to a VZP policyholder who meets the conditions set out for the specific programme and has no outstanding debts in the form of unpaid premiums, penalties, or related charges under the public health insurance system (including cases involving an unidentified payer of insurance).
  • The contribution cannot be drawn by persons who are exempt from the public health insurance system or for whom VZP serves only as a substitute health insurance company, holding a yellow insurance card.
  • When submitting an application or drawing a contribution, the policyholder must not be registered for transfer to another health insurance company as of the date of submission.
  • For clients who become VZP policyholders from 1 July 2026, the proof of payment required for granting the contribution must be issued and paid after registration with VZP (i.e., no earlier than 1 July 2026).
    For contributions where no payment documents are submitted, the date of the provided service must also fall after the client’s registration with VZP (i.e., no earlier than 1 July 2026).
    This applies to the following contributions:
    • Increasing the Availability of Psychosocial Support (date of recorded session),
    • Vaccination (date of vaccine administration),
    • Ski Course (date of course completion), and
    • Educational Stays for Diabetic Patients (date of stay completion).
  • A contribution may be requested until the set financial limit is reached,
    • once per year, if the contribution amount is up to CZK 1,000 (inclusive);
    • twice per year, if the contribution amount is over CZK 1,000.
      (Exceptions include contributions for Diet Programmes — once per half-year, Increasing Availability of Psychosocial Support — may be claimed after each session, and Vaccination — exhausted upon the first application.)
  • Payment documents with a payment date from 1 January 2026 are accepted, provided that at the time of submission they are not older than 3 months from the date of payment.
    If multiple payment documents are submitted, the 3-month period is counted from the date of payment on the most recent document.
  • For contributions conditional on a preventive medical check-up (specifically contributions for Physical Activities and Dental Hygiene, and Rehabilitation Activities), payment documents dated from 1 January 2026 are accepted, provided that at the time of submission they are not older than 6 months from the date of payment.
    If multiple documents are submitted, the 6-month period is counted from the payment date on the most recent document.
  • The minimum contribution amount per application is CZK 100.
  • Applications supported by payment documents issued and paid from 1 January 2026 are recognised as part of the 2026 Prevention Fund — they do not apply to the completion of the 2025 period.
  • The rules and conditions for granting a contribution — particularly the age limits and validity period of documents — apply as of the date of submission, unless otherwise stated for a specific contribution.
    For vaccination contributions, the date of vaccine administration is decisive.
  • In the calendar year in which the policyholder turns 18 years old, it is not possible to combine within the same category contributions intended for children under 18 and adults from 18 years.
  • In the calendar year in which the policyholder turns 65 years old, it is likewise not possible to combine within the same category contributions intended for adults up to 65 and seniors from 65 years.
  • Payment documents may be aggregated up to the maximum limit set for each specific contribution. The validity period of such documents is assessed based on the date of the most recent payment for goods, services, or activities eligible under the programme.
  • Contributions from the Prevention Fund are not legally enforceable entitlements. Their availability does not establish an automatic right to receive the benefit.
  • Contributions will not be granted for activities or products that clearly do not meet the preventive purpose of the individual programmes.

What Contributions Do Not Apply To

Contributions do not apply to:

  • payment documents for rental services (e.g., rental of halls, gyms, courts, saunas, swimming pools, whirlpools, solariums),
  • payment documents for online activities or video courses, including online sports or rehabilitation programmes,
  • payments covering any co-payment or difference between the amount reimbursed by public health insurance and the provider’s price for goods or services (e.g., medical services, pharmaceuticals, or medical devices covered by public health insurance),
  • payment documents that have already been used within the current year to draw a contribution under another programme (a single payment document cannot be used to claim contributions under multiple programmes),
  • payment documents covering multiple persons where it is impossible to determine the individual amount for each beneficiary (e.g., family passes or multi-person entries),
  • activities paid via a MultiSport card,
  • purchases of gift vouchers, certificates, coupons, discount vouchers, or cheques (including those obtained via discount portals),
  • payment documents for the purchase of goods from foreign e-shops,
  • ambiguous, illegible, altered, or otherwise invalidated

Who May Apply for a Contribution

  • A VZP policyholder aged 15 or older.
  • A legal guardian of a VZP policyholder under 18 years of age, who submits the required documents — typically the child’s birth certificate.
  • An authorised representative — based on a power of attorney.
    The power of attorney does not need to follow a prescribed form or be notarised, but it must contain all required details.
    The document must clearly specify the particular contribution for which the application is submitted.
  • An institution to which the child has been entrusted to care, in particular children’s homes.
    An employee of such an institution submitting the application must provide documentation confirming the entrustment of the child into care and the authorisation allowing them to act on behalf of the institution.
  • An adoptive parent, guardian, or custodian, who must present the relevant court decision confirming the entrustment of the child into their care.
  • A foster parent, who must likewise submit the relevant court decision on the entrustment of the child into foster care.

How You Can Apply for a Contribution

You can apply in one of the following ways:

  • Online via the Moje VZP application

For the following contributions, the application may only be submitted through the online application:

      • For adults: contributions for Selected Rehabilitation Procedures and Spa-type Therapies linked to a screening examination.
      • For children: contributions for Physical Activities and Dental Hygiene (not conditional on a preventive medical examination by a paediatrician).
      • For adults: contributions for Physical Activities and Dental Hygiene (conditional on a preventive medical examination by a general practitioner).

This restriction does not apply to seniors aged 65+ and persons with sensory disabilities, who may apply using any of the methods listed below.

If you do not yet use Moje VZP, you can easily register:

      • Quick Registration – log in easily via BankID or Citizen Identity and start using the application immediately, or
      • Registration via the Online Form – for security reasons, you will need an activation code, which will be sent to you by registered mail within 1–2 weeks. The application can be fully used only after entering this code.
        If you prefer not to wait, you can register directly at any VZP client office.
      • Once registered, you can submit your application without having to re-enter your personal information.
        Parents may submit applications on behalf of their children after registering them within their existing Moje VZP account.
  • In person – by visiting any VZP client office.
    In this case, you only need to bring the required documents; there is no need to complete the Application Form in advance.
    You can also submit your application directly to the VZP mailroom, together with all required supporting documents.
  • By post – by sending the completed Application for a Contribution, along with all required supporting documents, to:
    VZP ČR, Regional Branch Prague, Na Perštýně 359/6, 110 00 Prague 1.

What You Must Submit When Applying for a Contribution

When applying via the Moje VZP application

Please attach:

  • a scan or photograph of the proof of payment for the service, goods, or activity, including all required details,
  • a scan or photograph of any additional required documents specified for the specific contribution (e.g., a doctor’s confirmation of diagnosis, vaccination confirmation, maternity record book, sworn statement, etc.).

When applying by post

Please submit the following:

  • the child’s birth certificate, if you are applying as the legal guardian of a VZP policyholder under 18 years of age,
  • a power of attorney, if applicable,
  • a relevant court decision confirming the child’s placement into care, if you are applying as a foster parent, guardian, adoptive parent, custodian, or an institution entrusted with the child’s care,
  • a completed and signed Prevention Fund Application Form,
  • a proof of payment for the service, goods, or activity, including all required details,
  • any additional required documents specified for the particular contribution (e.g., doctor’s confirmation of diagnosis, sworn statements, confirmation of vaccine administration, maternity record book, etc.).

When applying in person

Please bring:

  • your identity card (for identity verification),
  • the child’s birth certificate, if you are applying as the legal guardian of a VZP policyholder under 18 years of age,
  • a power of attorney, if applicable,
  • a relevant court decision confirming the child’s placement into care, if you are applying as a foster parent, guardian, adoptive parent, custodian, or an institution entrusted with the child’s care,
  • a proof of payment for the service, goods, or activity, including all required details,
  • any additional required documents specified for the specific contribution (e.g., doctor’s confirmation of diagnosis, sworn statements, confirmation of vaccine administration, maternity record book, etc.).

Requirements and Forms of Payment Documents

  • Contributions are provided only during the validity period of the programme and exclusively for documents proving payment for the relevant activity, service, or goods in Czech crowns (CZK).
  • A payment document may take the form of an invoice, receipt, till slip, cash receipt, confirmation, or similar document.
    If a copy, scan, or photograph of such a document is submitted, VZP ČR reserves the right to request the original payment document within 90 days from the date of application submission.
  • Payments made via employee benefit cards are accepted, provided that the submitted document clearly indicates the purpose and amount of the payment.
  • The document must be clearly legible, especially in the case of scanned or photographed documents submitted online.
  • The payment document must include the following:
    • the type of document submitted,
    • the scope and subject of performance, i.e. the name of the service, goods, or activity; where necessary, this information may be supplemented by the client (including their signature),
    • the date of issue or, where applicable, the date of payment,
    • the price in CZK,
    • identification details of the business entity (Company ID No., business name – printed, handwritten, or indicated by a company stamp).
      VZP reserves the right to verify the submitted payment documents against the activities listed in the public register of the supplier that issued the document.
    • the identification of the policyholder for whom the payment was made.
      In justified cases, the applicant may add the identification of the person concerned, including their signature, on the reverse side of the document.
      This does not apply to simplified payment documents (e.g. till receipts or slips) issued without a name, where the identification of the person is not required.
    • for cash payments, it must be clearly indicated that the payment has been made.
    • for cashless payments (e.g. bank transfer, payment card, cash on delivery, online payment gateway), where payment is not clearly shown on the document itself, an additional proof of payment is required — such as a bank statement extract, bank-issued confirmation, payment terminal receipt, or cash receipt.

How the Contribution Is Paid

  • Contributions are provided exclusively via bank transfer to the bank account indicated in the application form.
    • If the application is submitted by an authorised representative based on a power of attorney, the contribution is transferred to the bank account stated in that power of attorney.
    • If the applicant provides an invalid bank account number and the payment is subsequently returned to VZP’s account, VZP is not obliged to contact the applicant or notify them of this fact.
  • VZP reserves the right to modify or terminate programmes during the year once the allocated financial limit has been exhausted.
