Physical Activities and Dental Hygiene for Children up to 18 Years of Age

⚠️ Important: Applications can be submitted only through the Moje VZP application.

Payment documents with a payment date from 1 January 2026 are accepted, provided that at the time of application they are no older than 6 months (for multiple payment documents, the 6-month period is calculated from the most recent payment).
An exception applies to school skiing courses, where:

• Only the official VZP ČR “Confirmation of Participation in an Organised Ski Course” form is accepted, which also serves as proof of payment (payment dates from 2025 or 2026 are both accepted).
• The form is completed and confirmed exclusively by the school after the course has taken place.
• At the time of application, the course end date must not be older than 6 months.

The contribution can be claimed gradually, in up to two applications.

Preventive Check-up Bonus for Adults (18-65) - Physical Activities and Dental Hygiene

To reward policyholders who take care of their health and attend preventive check-ups with their general practitioner, VZP offers a contribution for:

If you have completed a preventive check-up with your general practitioner for adults, or with a general practitioner for children and adolescents (not a dental preventive check-up), within the last two years, you may apply for the contribution directly in the Moje VZP application.

If you have not yet had your preventive check-up, we recommend completing it first before purchasing a physical activity or booking a dental hygiene session.
Please note that the reporting and registration of the provided health care by your doctor or hospital can take up to 3 months.

The contribution can only be claimed with a proof of payment for a physical activity or dental hygiene session not older than 6 months at the time of application.

The Contribution Can Be Used For

• Regular, long-term physical and sports activities – memberships, passes, courses, entry fees (e.g. aerobics, badminton, floorball, football, callanetics, swimming, spinning, pilates, circuit training, yoga),
• Regular, long-term physical and sports activities provided by a physiotherapist (not for reimbursement of physiotherapy, rehabilitation, or reconditioning treatments),
• Dental hygiene treatment and instruction.

Preventive Check-up Bonus for Seniors (65+) – Physical Activities and Dental Hygiene

Policyholders who take care of their health and regularly attend preventive check-ups with their general practitioner, and who have completed a preventive check-up within the last two years (not a dental preventive check-up), may apply for a contribution towards:

If you have not yet attended a preventive check-up with your general practitioner, we recommend completing it first before purchasing a physical activity or booking a dental hygiene session.
Please note that the recording of medical services by the doctor or hospital can take up to 3 months.
The contribution can be claimed only with proof of payment for a physical activity or dental hygiene service not older than 6 months at the time of application.

The Contribution Can Be Used For

• Physical, recreational, or sports activities (e.g. Sokol, hiking groups, yoga),
• Health-promoting exercise,
• Physical and sports activities provided by a physiotherapist (not for reimbursement of physiotherapy or rehabilitation),
• Membership fees and competition entry fees,
• Dental hygiene treatment and instruction.

Adults aged 18 and above who have undergone a preventive screening examination (colorectal, cervical, mammography, lung, or prostate screening) may apply for a contribution towards:

• Massages
• Swimming
• Physiotherapy
• Sauna
• Cryochamber
• Salt cave
• Therapeutic baths and hydrotherapy
• Therapeutic wraps
• Shockwave therapy
• Laser therapy
